To request an Emergency Medical / Ambulance Report please click here for a request for information form fill it out and return it to the address below. There is no charge for an Emergency Medical / Ambulance report.

Fire Reports

Please download and fill out the incident request form or provide the following information in a written request:

• Incident date  

• Approximate time of the incident
  

• Address or location of the incident

• Address to send report to

• Name of requester

A fire report fee of $5.00 must accompany the request.
